   1. 20 CPD Hours for those based at Karachi, Lahore and Islamabad.
   2. 10 CPD hours for Members/Qualified Students based at cities/centers beyond the radius of 50 KM of Karachi, Lahore and Islamabad
   3. 05 CPD Hours for overseas-based Qualified Students.

1. Duly filled-in Application Form with
   1. Two copies of passport-size coloured photographs
   2. One copy of ICMAP Passing Certificate
2. Three years relevant experience certificate(s), from current as well as previous employer(s).
3. CPD Credit Hours certificate(s), if programmes attended are of organizations other than ICMAP
4. Three Recommendation letters from ICMAP members OR two from members of ICMAP and one from other executive. Only names, membership numbers and addresses may be mentioned on the form, rest of the formalities will be completed by the Members' Section/Centres
5. 1 set of Resume

1. Application Fee = Rs.3,000/- + Annual Subscription in advance as Associate Member i.e. Rs.4,000/- for 2012-13, Total Rs.7,000/-
2. Annual Subscription(s) as qualified student, starting from the proceeding financial year of qualification (if applicable).
